Primary Care Partnerships to Prevent Heart Disease in Women

Primary Care Partnerships to Prevent Heart Disease in Women (PCP) is a CME program, focused on the Evidence-Based Guidelines for Cardiovascular Disease Prevention in Women, sponsored by the U.S. Department of Health and Human Services, Office on Women’s Health as part of The Heart Truth campaign. The goal of PCP is to increase awareness among primary care providers in the high-risk Upstate and Western New York areas regarding how to assess, diagnose, and treat heart disease in women.
This program is being conducted by the Research Center for Stroke & Heart Disease of the Jacobs Neurological Institute at the State University of New York at Buffalo which works with clinicians and community members to raise awareness of and reduce risk for stroke and heart disease through the design, implementation, and evaluation of evidence-based programs.

Heart Truth Ohio

The Heart Truth Ohio program provides various tools and techniques needed to counsel women on risk factors and advice on making heart-healthy lifestyle choices.

The goal of this initiative is to increase knowledge, counseling skills and treatment of women according to American Heart Association Guidelines for the Prevention of Cardiovascular Disease in Women.


The American Heart Association is the largest voluntary health organization fighting heart disease, stroke, and other cardiovascular diseases, which kill more than 945,000 Americans each year. In February 2004, the American Heart Association (AHA) launched a complementary campaign, “Go Red for Women,” designed to mobilize women to take control of their heart health and invite women to make their health a top priority and live a stronger, longer life. The Delaware Academy of Medicine was founded in 1930 to address the ever evolving advances in medicine, dentistry, and public health. LaRedHealthCenterlogoLa Red Health Center (LRHC) represents the efforts of a multi-year collaboration among a variety of organizations responding to increasingly poor health statistics in the federally designated rural county of Sussex, Delaware.  Their priority is to provide health services to Sussex County residents and seasonal/migrant workers who have difficulty affording and accessing health care. LRHC provides a myriad of health and social services to the medically under- and uninsured in Sussex County. For over eight years, LRHC has relied on its established administrative infrastructure and strong relationships with other community-based, public- and private sector partners to bring vital health care services to the County’s neediest citizens.   LRHC received federally qualified health center (FQHC) designation in 2005 and federal funding in 2006.

LRHC focuses on the following problem areas: prenatal care; immunizations; heart disease; cancer; diabetes; and HIV.  In addition to primary care services for the full life continuum, LRHC offers health promotion and disease prevention education, financial and social services case management, outreach and transportation.

Westside Family Healthcare is a community health center that provides family medical care, prenatal and maternity care, dental care, and integrated behavioral health care and has locations in Wilmington and Newark, with a new office opening in the Bear/New Castle area in July 2009.  Westside has been in operation since 1988 with the mission to provide equal access to quality healthcare, regardless of ability to pay.  Westside Family Healthcare employs a highly qualified staff of more than 130 employees, the majority of whom are bilingual in English and Spanish. Westside’s services include family medical care; women’s health care, including prenatal care and delivery, family planning and gynecological care, and on-site mammography; disease management and prevention for cancer, cardiovascular disease, and diabetes; adolescent health services; dental care; podiatry care; mental health care; pediatric immunizations; case management; certified diabetes education; bilingual (Spanish/English) care and translation; financial aid assistance; laboratory services; low cost medications; and 24-hour emergency response.  Westside accepts most private insurances as well as Medicare, Medicaid, Delaware Physician’s Care, Inc., Unision and Sreening for Life.  Westside offers a sliding fee scale for those who are uninsured, based on their income. Westside Family Healthcare has been a Joint Commission accredited facility since 2000.

 

Beebe Medical Center, located in Lewes, Delaware, is a community-owned, not-for-profit hospital with 210 licensed beds. It offers progressive and comprehensive medical care that includes an open-heart surgery program, which is an affiliation with Christiana Care Health System, cardiac intervention, cancer treatment, orthopedic surgery, women’s health and emergency medicine. It also has numerous outpatient facilities in satellite locations around eastern Sussex County offering services including imaging and lab services and physical and occupational therapy, and a school of nursing adjacent to the main campus. More than 200 physicians who bring more than 30 medical specialties are members of the Medical Staff.
